He drew the lines himself on blank paper. I'm not sure when printed manuscript became available but certainly it was available to Beethoven.

Ohhh! So Bach and possibly Mozart during part of his life had to do a lot of work before using the paper!

Judging from the manuscripts of Bach and other composers from around then, I would guess they had some sort of tool that worked like five equally spaced pens, so they could draw an entire staff at once, and not have to draw each line individually. Their staves are often curved or crooked, but all the lines seem to follow the same shape.
